BY CHOOSING TO WORK WITH CARL EDEN HYPNOSIS YOU AGREE TO THE FOLLOWING:

1. You understand the scope of the therapies Carl Eden provides and give your full consent to receiving therapy sessions from Carl. You understand that results may vary from person to person and, as with any therapy, a ‘cure’ is not guaranteed.

2. You understand that hypnotherapy provided by Carl is not a replacement for medical treatment, psychological or psychiatric services. You also understand that Carl does not treat, prescribe for, or diagnose any medical condition. Furthermore, any comments made by Carl should not be taken as medical advice and will be made only in response to your comments, beliefs and understandings. 

3. You declare that, if advised prior to any session with Carl to seek medical approval, you have consulted with your General Practitioner and/or Hospital Consultant and gained the appropriate medical approval for working with Carl.

4. Confidentiality is paramount and will be maintained in all but the most exceptional circumstances. You agree that these can include: legal action (criminal or civil court cases where a court order is made demanding disclosure, including coroners’ courts); child abuse; if there is imminent danger to Carl or others; and where there is good cause to believe that not to disclose would cause danger of serious harm to others.

5. A deposit is always required to confirm a booking. If the required deposit is not payed within 24 hours of booking then the appointment will be cancelled. Deposits are non-refundable should you cancel or miss your appointment.

6. In the event that you need to reschedule your appointment then a rescheduling fee of £20 will be charged. Furthermore, Carl operates a strict policy of forty-eight hours notice required to cancel or reschedule an appointment. If this is not given, he reserves the right to charge the full session amount.

7. In the unlikely event that Carl needs to reschedule your session then he will endeavour to inform you as soon as possible and will offer you an alternative appointment to suit you or a refund for that session after which his liability will be at an end.

8. Any written information given by you will be dealt with in accordance with the General Data Protection Regulation (GDPR) and UK law.

9. By returning your Client Intake Form you agree with the terms and conditions and are confirming that all the details you have given are true and correct.

10. The maximum time this information will be held is 7 years. You have the right to ask for this information to be deleted at any time and can do so by requesting deletion of information to carl@edenhypnosis.com

11. Carl reserves the right to decline or discontinue working with any client without having to declare any reason. For example, but not limited to, if you arrive for your appointment intoxicated or under the influence of recreational drugs, Carl reserves the right to stop the appointment or decline seeing you.
